What actually IS a “heart murmur”?

I’ve had multiple small dogs diagnosed with a heart murmur. Some have been given medication, some have not, but none seem to be actually affected by it. What is it, really, and is medication required always, sometimes, never?

A heart murmur is actually a leaking heart valve. The leak will make a swishing sound as the heart beats that is referred to as a murmur. The severity of the leak will determine if treatment is required. If severe, fluid will accumulate in the lungs and or the abdomen resulting in clinical symptoms. These cases will require medication.
