Fear of noises

Q (because her sister was Suzie–get it?) is almost 13, and I have had her 12 of those years. She has suffered from noise fears all her life. She will run away, go over brick walls, try to open door handles, and tear up my house to get away. One time she went over the wall which is about 4 feet on my side and 6 feet on my neighbors. She went through their dog door and jumped into bed with the man who lives there. This is sort of typical behavior or her. She is agility trained and VERY smart. I can always repair the house, but not my dog. She is on Rymadyl, Amatriplyn, Tradnazone, and Melatonin. They help. Is there anything else I can try?

For a known event, such as July 4th or a pending thunderstorm, the use of acepromazine can be helpful. For the long run, a consultation with an animal behaviorist or an animal communicator would be helpful.
