I have a question about…persistent rash

My golden retriever (almost 13 yrs old) has a persistent rash that comes about every 3 months (it gets crusty in spots especially on his underside near his hind legs and then causes hair loss as well). The vet always puts him on simplicef which does work but after the treatment of antibiotics it comes back about 2-3 months later. We have since put him on Science diet prescription Z/D Ultra which still does not seem to make a difference either. This has been going on for about 3 years; we bathe him 2x a week as directed with ketachlor shampoo when he gets the rash. He has been blood tested and skin tested and each time they only find slight yeast and/or bacteria on his skin and tell us that its not enough to be causing his rash. He was also diagnosed with a tumor on his spleen in dec. 2010 but that does not seem to have any affect on him. PLEASE help! I’m at my wits end with the scratching and rash and the vet seems to be of no help!
