This sounds like paraphimosis, a constriction of the sheath opening. When confronted with this we initially try frequent topical application of an anti-inflammatory lubricant, such as panolog, and keep the penis pushed into the sheath. If the problem persists, surgery to enlarge the sheath opening may be required.
